Local excision of rectal cancer after neoadjuvant treatment

Abstract

Over the past decade, we have observed the emergence of organ preservation treatment for patients with rectal cancer. This trend has developed from two separate sources, firstly, local excision of early stage rectal cancer utilizing platforms such as transanal endoscopic microsurgery (TEM) and, secondly, use of neoadjuvant chemoradiation for treatment of more advanced rectal cancer to achieve complete clinical response (cCR). TEM may be utilized to achieve local resection of small localized rectal tumours, without removing lymph nodes; however, the risk of local recurrence prohibits widespread adoption of this strategy beyond very early-stage disease [1, 2].
