Abstract

Background: The 2018 updated classification of periodontal diseases led to the development of new evidence-based guidelines to ensure precise and effective treatment of periodontal and peri-implant diseases. The German S3 guideline for the treatment of stage I to III periodontitis is based on recommendations of the European Federation of Periodontology and forms the foundation for the forthcoming adaptation of guidelines for stage IV.

Objective: The aim of this work is to outline the evidence-based steps in periodontal therapy as per the S3 guideline, including diagnostic, risk management, and treatment recommendations.

Material and methods: The S3 guideline was developed through systematic literature review and expert consensus. The described therapeutic phases and recommendations are based on internationally recognized studies and meta-analyses of very high methodological quality.

Results: Periodontal therapy comprises four phases: 1. behavioral therapy to improve oral hygiene and control risk factors, 2. causal therapy focusing on subgingival biofilm disruption, 3. specific interventions for non-responders, and 4. lifelong supportive periodontal therapy (SPT). Stage IV patients require a multidisciplinary approach to restore functionality and aesthetics.

Conclusion: The S3 guideline provides an evidence-based framework for the structured treatment of periodontitis. Accurate diagnostics and individualized treatment are essential to ensure long-term therapeutic success and stable oral health.
