Abstract

Person centred planning aims to create a visual picture of the life of a child/young person in order to gain an understanding of their, and their family’s, values and needs and how they can best be met. It is a simple and effective approach to assessment that can result in comprehensive, informative documentation.
Practical applications informing care provision include; providing a clear focus for discussions between the child/young person and their care providers empowering their active involvement in care planning, and providing health professionals with a framework to support, and inform challenging conversations.
